DUH Certificate of Need Letter of Support

Step-by-Step Instructions:

1. Download the template Letter of Support (below)
2. Edit the letter - or leave it as is
3. Add an electronic signature or print and sign your name, including title and specialty
4. Submit your signed letter to Catharine Cummer, DUHS Regulatory Counsel, BY THIS FRIDAY, OCTOBER 13 at NOON   

All letters will be compiled and submitted along with the application.

Three Ways to Submit Your Letter:

• Email the signed letter to Catharine.Cummer@duke.edu 
• Fax it to 919.419.5001
• Deliver to the DHIP Administrative Office, 1287 Orange Zone, Duke South
